A. Individuals performing monitoring and/or maintenance activities as described in Chapter 246-272A WAC, Snohomish Health District Sanitary Code and/or in the various Recommended Standards and Guidance documents pertaining to OSS published by the Washington State Department of Health shall report the activities to SHD within 30 days of completion in the manner herein described.

B. SHD will maintain an electronic database for the purpose of submitting reports related to monitoring and maintenance of OSS as well as limited repair measures.

C. Reports must be submitted electronically and in the format prescribed by SHD. At a minimum, reports must include the following:

1. Address of property.

2. Parcel number of property.

3. Owner of property.

4. Description of work performed, including:

a. A complete and thorough evaluation of each system component.

b. Any adjustments or calibrations performed on the system and related components.

c. Limited repair measures as described in SHDC 5.70.170.

d. Replacement or repair of proprietary system components.

5. Name and certificate number of person performing the work.

6. Signature and date.

D. Reports must be accompanied by the appropriate fee as determined by the current fee schedule. [Res. 12-15. Prior code § 8.7 (Supplement)(VI)].
