This application is to be used by the resident owner requesting a permit to repair an existing on-site sewage disposal system who wishes to participate in the site evaluation preparatory work and site plan drawing. This application can be submitted by the resident owner only. Repair applications for rental homes, commercial establishments and multifamily units must be submitted by a certified designer or engineer.

The Snohomish Health District (SHD) will assist the resident owner in the design of conventional on-site sewage disposal systems only.
